Key Duties

• Conduct psychosocial assessments and interventions for inpatients and emergency department patients.

• Deliver crisis intervention, counseling, and resource referrals for patients and families.

• Collaborate with case managers, nurses, physicians, and payers to coordinate patient needs.

• Develop safe discharge plans and assist with post-acute care placement.

• Support patients and families with end-of-life care, chronic illness, and emotionally challenging situations.

• Stay current with Medicare, Medicaid, and insurance payer regulations to ensure compliance.

Requirements

• Master’s degree in Social Work (MSW) required.

• Strong knowledge of psychosocial assessment, crisis intervention, and discharge planning.

• Ability to work collaboratively within a multidisciplinary healthcare team.

• Excellent communication, counseling, and problem-solving skills.

• Familiarity with managed care and length-of-stay processes preferred.
