Department: Social Work / Patient Services

Conduct biopsychosocial assessments and provide non-clinical interventions for patients on inpatient floors and in the emergency department.

Assist with crisis intervention, emotional support, and high-risk patient assessments.

Coordinate post-acute care and discharge planning for patients, addressing behavioral health and resource needs.
-
Collaborate with interdisciplinary teams including nurses, physicians, case managers, and insurance representatives.

Identify and resolve barriers to discharge and recovery, including financial, psychosocial, and medical challenges.
-
Facilitate meetings with patients, families, and caregivers to support care decisions and advocate for patient needs.
-
Provide guidance and support for end-of-life, terminal illness, and crisis situations.
-
Master’s Degree in Social Work (MSW) required.
-
Board eligibility or licensure in the state of Florida preferred.

Strong knowledge of Medicare, Medicaid, managed care regulations, and discharge planning processes.

Excellent communication, interpersonal, and problem-solving skills.

Ability to provide compassionate patient and family-centered care in high-stress environments.
