As a Care Management Coordinator at Northwell Health, you will provide essential support to children and families enrolled in the Health Home Serving Children program, a New York State Medicaid initiative. Your role will focus on ensuring coordinated care that addresses both medical and social needs. Core responsibilities include:
- Building rapport with patients and families through motivational interviewing and patient-centered engagement techniques.
- Collaborating with primary care teams to manage patient panels, reduce gaps in care, and promote follow-up adherence.
- Creating, maintaining, and updating individualized care plans for high-risk children, considering both medical and social barriers.
- Acting as a liaison between hospitals, specialists, primary care providers, and community services to facilitate seamless care coordination.
- Monitoring special populations, including patients discharged from inpatient or emergency services, and tracking preventive or chronic care needs.
- Performing outreach in various settings, including homes, primary care sites, hospitals, and neighborhoods, to ensure continuity of care.
- Closing communication loops on referrals, follow-ups, and outstanding care plan tasks using team-based strategies.
- Bachelor’s degree required; Master’s in Social Work or related field strongly preferred.
- 1–3 years of relevant experience in home health, care management, or related field; prior experience with pediatric populations preferred.
- High school diploma or equivalent required.
- NYS Driver’s License preferred.
- Strong interpersonal, organizational, and communication skills.
- Ability to work effectively in a hybrid schedule, including fieldwork in multiple NYC boroughs and Nassau County.
