Key Responsibilities | Support meetings, capture outcomes, coordinate actions, liaise with key workers |
Stakeholder Engagement | Work with health, social care, and voluntary sector professionals |
MDT Support | Assist INT MDT with triage, decision-making, and evaluation of patient information |
Flexibility | Cover for colleagues across Localities as needed |
Reporting | Maintain accurate records and documentation |
Communication Skills | Clear verbal and written communication with multi-disciplinary teams |
Organisational Skills | Highly organised, self-motivated, and proactive |
IT Competency | Comfortable using IT systems for scheduling, reporting, and record-keeping |
Professional Behaviour | Reliable, flexible, and supportive in collaborative settings |

Travel Requirement | Must be able to travel across sites within Birmingham and Solihull |
